Scope: 100% adult Hospice/Palliative care – Remote position

Duties/Volume: not seeing patients via telehealth the entire time (8am-5pm). It is covering the service/providing clinical care of 130-150 patients; however, this is done through the nurses seeing the patients in their home or facility and contacting physician. Working with hospital liaison and intake on reviewing chart to see if someone is appropriate and giving orders for medications on admission. Doing certificate of terminal illness ( CTI) the day after a patient is admitted or on Monday for those admitted during weekends. CTI will require review of hospice admit RN note and review of medical records in EPIC. Medications review with hospice covered and non-covered medications at time CTI is done on all new patients. This is based on hospice preferred medication list. Med review note in chart. 2-6 admissions /day. Fewer on weekends. Possible phone calls or telemedicine visits with patients for goals of care or symptom management. These almost always happen when RN in home and use their phone for calls/videos. Assessing appropriateness for GIP on inpatients. Working with hospice team and hospitalist to coordinate care for GIP patients. IDT meetings required and occur every Tuesday morning and every other Thursday. Signing prescriptions for hospice patients. Signing faxes for orders for facilities. Experience with starting and titrating opioid infusions, ketamine and methadone. Occasional face to face visit for admission (most other administrative F2F visits can be done by our NP or other physicians).
